Disparities in referrals for home health care affect Black people living with serious illness, even with similar discharge scores.

A journal article published in Medical Care sought to examine differences in referral to home health care (HHC) services at the time of hospital discharge, based on a patient's race and ethnicity. The authors found that although Black patients scored similarly on the Patient's Readiness for Hospital Discharge Scale (RHDC), they were less likely to receive a referral to HHC services, to provide essential continuity of care.
