Embedding Palliative Care in State Health Reform Initiatives
The National Academy for State Health Policy (NASHP) just released a new brief describing strategies that states can use to embed palliative care into existing programs that serve residents living with serious illness. Programs and services include Medicaid 1915(c) Home and Community-Based Services Waivers; Medicaid Managed Care; Medicaid Managed Long-term Services and Supports (MLTSS); Dual-Eligible Special Needs Plans (D-SNPs); Patient-Centered Medical Homes (PCMH); and Medicaid Health Homes. The brief also includes examples from Arizona, California, Colorado, Oregon, South Carolina, and Washington. This work is part of a larger project to advance palliative care through state policy, supported by The John A. Hartford Foundation.
